Seeking Specialized Occupational Therapist (Adult Executive Dysfunction & Trauma-Informed)

Location: Las Vegas, NV

I am currently seeking a highly skilled Occupational Therapist who specializes in executive dysfunction, neurodevelopmental delay, and trauma-informed care. Specifically, I am in search of an OT who understands how to build external supports for impaired executive functioning, rather than relying on insight, willpower, or verbal strategies alone.

A little about the patient (my partner): adult female with documented frontal–subcortical executive dysfunction, ADHD, trauma-related limbic dysregulation, dissociative states, and developmental delay. As a result, she has significant difficulty with:

  • ·         Task initiation and completion
  • ·         Sequencing and transitions
  • ·         Time awareness and prospective memory
  • ·         Basic self-care routines (teeth, showering, medications, getting started)
  • ·         Emotional regulation under demand
  • ·         State-dependent judgment and vulnerability when overwhelmed.

A prospective OT candidate should have experience or strong interest in adult executive function impairment, neurodevelopmental delay, ADHD with complex trauma, brain injury / hypoxic injury effects, dissociation-informed or autonomic-informed care, and sensory processing and visual/sensory fatigue. In terms of clincal approach, I am seeking

  • ·         Externalized executive supports (visual, tactile, environmental)
  • ·         Task initiation and sequencing scaffolds
  • ADL support that reduces cognitive load
  • Proceduralization of routines (not just teaching steps)
  • State-based planning (working with regulation, not against it)
  • Real-world, practical intervention (not just discussion)

Conversely, if you rely primarily on verbal instruction or insight, shame or compliance-based models, emphasize motivation/accountability, expect linear progress, or view dissociation as avoidance, this may not be a good fit for you.

As it stands now, I am looking for daily support in the mornings and evenings, at least initially. That said, this is expected to be a long-term in-home support role. I also welcome collaboration with the rest of my health care team. With respect to payment, she has BCBS Anthem Silver EPO insurance, and we are open to private pay.
